Responsibilities
  • Assess and coordinate patient discharge planning needs with healthcare team members.
  • Perform point-of-care testing according to established policies and procedures.
  • Collaborate with physicians to develop and implement patient care plans, ensuring timely and accurate documentation.
  • Coordinate and communicate with multidisciplinary team members and outside agencies to achieve optimal patient outcomes.
  • Provide skilled nursing care within established care plans, including vital sign monitoring, wound care, IV management, and physical assessments.
  • Document visit notes, treatments, and care in a timely and accurate manner, adhering to regulatory requirements.
  • Respond to patient and family inquiries, addressing needs, issues, or concerns in a professional and empathetic manner.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • Valid driver's license and a satisfactory driving record.
