Overview

We are seeking a highly skilled and compassionate Medical Social Worker to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a Medical Social Worker, you will play a vital role in providing patient-centered care and support to individuals and their families in the comfort of their own homes.

Responsibilities
  • Assist patients and families with health-related financial, social, and emotional concerns, and provide effective coping skills.
  • Provide social work services in accordance with the plan of care, ensuring seamless coordination with other healthcare team members.
  • Educate patients, families, and healthcare team members on medical social work interventions and treatment plans.
  • Perform care planning, transfer, and discharge functions, identifying and mitigating risks to patient health.
  • Communicate effectively with patients, families, and healthcare teams to coordinate care and ensure optimal outcomes.
Qualifications

To be successful in this role, you will need:

  • A master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
  • One year of social work experience in a healthcare setting.
  • Current CPR certification.
  • Meet state and federal requirements for this position.
