About the Role:
DaVita is seeking a skilled Social Worker to join our team. As a Social Worker at DaVita, you will be a vital member of each patient's core care team, providing support and advocacy to patients with End Stage Renal Disease.
Key Responsibilities:
- Help patients understand their rights and responsibilities
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease
- Build meaningful and long-term relationships with patients and their families
- Be a part of a team that appreciates, supports, and relies on each other in a positive environment

Requirements:
- Meets all state required regulations to practice in the dialysis setting
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, as well as functional proficiency with DaVita-specific applications within 60 days
