Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Hudson Headwaters Health Network. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Key Responsibilities
  • Provide age-appropriate, population-specific patient care in a professional, compassionate, and culturally sensitive manner.
  • Administer care to patients under the direction of the responsible Registered Nurse or medical provider.
  • Collaborate with staff and other departments to develop strategies for continuous quality improvement and patient satisfaction.
  • Utilize data to meet care coordination needs across settings, working with patients, families, and other care providers.
  • Protect patients and employees by adhering to infection control policies and protocols.
  • Prepare patients for encounters by following established protocols.
  • Demonstrate competence in clinical skills and proper medication management processes.
  • Reconcile medication lists and prepare prescription renewals.
  • Document patient information accurately and timely in the electronic medical record.
  • Respond to patient phone calls and engage providers or RNs as needed.
  • Reinforce patient education plans to meet individual needs.
  • Perform point-of-care testing, including specimen collection and documentation.
  • Assist with patient flow and troubleshoot issues in collaboration with supervisors.
  • Provide guidance and mentorship to new and incumbent staff.
  • Attend meetings and serve on organization committees.
  • Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
  • Complete daily, weekly, or monthly tasks as assigned.
  • Ensure operation of medical equipment according to manufacturer instructions.
Requirements
  • Unrestricted current LPN license to practice nursing in New York State.
  • Certificate from an accredited school of nursing.
  • Minimum of 1 year of clinical experience preferred.
  • Ability to travel throughout the Network.
  • Current BLS certification.
  • Proficiency with Microsoft applications and electronic medical records.
  • Excellent verbal and written communication skills.
  • Self-directed, detail-oriented, motivated, and able to work independently and in a team atmosphere.
Compensation

The pay rate for this position is $28.00/hour.
