Job Description:

We are seeking a compassionate and dedicated Home Health Nurse (RN) to provide high-quality healthcare services to patients in their homes. Responsibilities include assessing, planning, implementing, and evaluating individualized care plans for patients with various medical conditions.

Key Responsibilities:

  • Provide in-home direct patient care according to the provider's prescribed plan of care
  • Assess and document patient conditions, report changes as needed
  • Administer medication, insulin, and IV/fluids, documenting thoroughly
  • Care for wounds, change dressings, and assist with personal hygiene
  • Collaborate with providers, aides, and allied health professionals

Requirements:

  • High school diploma or equivalent
  • Oasis experience
  • Minimum 12 months of experience as a Home Health Nurse (RN)
  • Strong communication and interpersonal skills
  • Current BLS certification
  • Two positive work references
  • Background check and drug test
