Occupational Therapist Job Description

Angels of Care Pediatric Home Health is a clinician-owned and operated home health agency dedicated to serving the special needs community. We are seeking a highly skilled Occupational Therapist to join our team.

Job Summary

We are looking for an Occupational Therapist to implement treatment programs to assist pediatric patients with physical, cognitive, sensory, and social/emotional disabilities or delays. The successful candidate will plan and administer occupational therapy in the home and community, providing high-quality care and meeting the needs of patients and their families.

Responsibilities
  • Perform evaluations and interpret assessment results to develop goals appropriate for each child.
  • Create and implement occupational therapy treatment plans in conjunction with the physician.
  • Assist pediatric and young adult patients in areas such as physical, neurological, cognitive, sensory, feeding, ADLs, and social/emotional development.
  • Implement skilled interventions to maximize the potential of each individual child.
  • Participate in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine.
  • Coordinate with referral partners to provide services for children in accordance with the physician order.
  • Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.
  • Document patient care services and care coordination in an intuitive electronic medical record system.
  • Maintain patient confidence by keeping information confidential.
Requirements
  • State license
  • Current CPR certification
  • A minimum of 1 year of experience preferred
