Job Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in Middletown, Ohio. As an LPN with IntelyCare, you will have the flexibility to choose your own shifts and facilities, allowing you to maintain a healthy work-life balance.

About the Role:
  • Work in post-acute facilities in the area as an LPN
  • Follow and adhere to resident care plans
  • Monitor residents' wellbeing, both physical and mental, and track vital signs
  • Conduct intakes, document health concerns and medical history of residents
  • Administer and verify medications, monitoring their effects
  • Review residents' charts and medical records
  • Assist in maintaining a safe, comfortable facility environment
  • Maintain appropriate stocks of medical supplies
  • Participate in IV set up, immunizations, and phlebotomy procedures
Requirements:
  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
