  • Job Summary: We are looking for a highly motivated and dedicated Physical Therapist to provide physical therapy services to patients in their homes, promoting independence and improving quality of life.
Responsibilities:

As a Physical Therapist with Care Partners, your key responsibilities will include:

  • Providing Comprehensive Physical Therapy Services: Assess, evaluate, and develop individualized plans of care to address each patient's specific physical therapy needs.
  • Evaluating Patient Progress: Regularly assess and document patient progress, making adjustments to treatment plans as needed.
  • Collaborating with Interdisciplinary Teams: Work closely with nurses, social workers, and other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
  • Communicating Effectively: Maintain open communication with patients, families, and caregivers, ensuring they understand treatment plans and goals.
  • Maintaining Accurate Records: Document patient interactions, treatments, and progress in a timely and accurate manner.
Qualifications:

To be successful in this role, you will need:

  • Graduation from an Accredited PT Program: A degree in Physical Therapy from an accredited institution is required.
  • Licensure and Certification: Current licensure as a Physical Therapist in the state where you will be practicing, along with current CPR certification.
  • Experience: At least two years of experience as a Physical Therapist, preferably in home health or community settings.
  • Excellent Communication Skills: Strong verbal and written communication skills, with the ability to effectively interact with patients, families, and interdisciplinary teams.
