Key Responsibilities:
- Seeking a full-time Nurse Practitioner or Physician Assistant (APC) to enhance our team in Utah.
- Engage in a diverse range of tasks, including conducting Annual Wellness Visits and serving as a provider billing educator.
- Perform Annual Wellness Exams for our Medicare Advantage patients at various Optum Utah Wellness Clinics, including community centers and mobile clinics.
- Utilize clinical expertise to compile a comprehensive list of current and historical medical conditions based on your assessments.
- Effectively communicate your findings to inform primary care providers about potential care gaps.
- On average, you will interact with 10 Medicare patients daily during clinic hours.
- Conduct educational one-on-one discussions with patients on topics such as disease management, medication adherence, and more.
- Act as a provider educator on the value-based care model, focusing on HCCs and Medicare HEDIS/Stars measures.

Required Qualifications:
- Active Nurse Practitioner or Physician Assistant license in Utah, along with national certification in Family Medicine, Adult Medicine, or Geriatrics.
- Possession of an active DEA license with prescriptive authority, or the ability to obtain one.
- Strong proficiency in patient assessment and education.
- Valid Utah Driver's License and auto insurance in compliance with state regulations.
- Capability to work independently.
Preferred Qualifications:
- Minimum of 2 years of clinical experience as a Nurse Practitioner or Physician Assistant.
- Experience in specialties such as Gerontology, Cardiology, Internal Medicine, or Endocrinology.
- Familiarity with the Medicare Advantage population and conducting patient assessments/annual wellness exams.
- Understanding of value-based care models, including HCCs and Medicare HEDIS/Stars measures.
