Job Title: Nurse Care Manager

At Innovative Senior Solutions Inc, we are seeking a highly skilled and compassionate Nurse Care Manager to join our team. As a key member of our Mobile Adult Day Health (ADH) Centers, you will be responsible for managing client health services, coordinating care, and leading staff and volunteers.

Key Responsibilities:

  • Client Health Services:
    • Conduct thorough assessments, manage medications, and develop holistic care plans for each client.
    • Oversee therapeutic activities, personal care, and support for activities of daily living (ADLs).
    • Coordinate healthcare services with client families, physicians, and case managers.
  • Staff and Volunteer Management:
    • Supervise, train, and support caregivers, engagement specialists, and volunteers.
    • Cultivate a supportive environment by fostering teamwork and collaboration between staff and volunteers.
  • Operational Oversight:
    • Manage daily operations, including maintaining required documentation, staff and volunteer schedules, and other tasks as required.
    • Maintain a safe and engaging environment for all clients.
    • Travel to each mobile site to ensure consistent quality of care.
  • Community Engagement and Volunteer Coalition:
    • Cultivate a robust volunteer coalition that engages with center participants, fostering a sense of community.
    • Encourage volunteers to bring unique activities and programs that enhance client experiences.

Qualifications:

  • Licensed Practical Nurse (LPN) license in Georgia.
  • Experience in adult day health services, home health, or elder care.
  • Strong leadership, team, and volunteer management skills.
  • Knowledge of dementia care, healthcare regulations, and senior care best practices.
  • Ability to travel to various mobile ADH sites as required.
