Responsibilities- Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
- Conduct thorough health assessments, develop and implement individualized care plans, and coordinate care with other healthcare professionals as needed.
- Order and interpret diagnostic tests, prescribe medications, and provide education to patients and their families on health-related topics.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Stay up-to-date with the latest medical research, guidelines, and best practices in family medicine.
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
- Current licensure as a Nurse Practitioner in the state of Wisconsin.
- Board certification in Family Medicine or a related specialty.
- Minimum of 2 years of experience as a Family Practice/Primary Care Nurse Practitioner.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
