Job Summary:

Key Responsibilities:

• Provide physical therapy services as prescribed by the physician that can be safely provided in the home.
• Conduct initial evaluations, and admission visit within 48-72 hours of referral for patients being admitted for physical therapy; in addition, develop a plan of care.
• Collaborate with a multidisciplinary team, including physicians, nurses, case managers, OT, to coordinate patient care and optimize treatment outcomes.
• Provide overall supervision of physical therapy assistants.
• Deliver evidence-based physical therapy interventions, including exercises, manual techniques, and modalities, to improve patients mobility, strength, and overall functional abilities.
• Maintain accurate and thorough documentation of patient progress in compliance with regulatory requirements.

Requirements:

• Current licensure to practice Physical Therapy in the state you are working.
• Possess and maintain a valid Driver's License required.
• Ability to work independently while adhering to established clinical protocols and guidelines.
• Excellent oral and written communication skills.
• Excellent organizational and time-management abilities to effectively manage caseloads/meet deadlines.
• Possess the physical ability to perform job related duties, which may require sitting, lifting, standing, bending, stooping, stretching, walking, pushing, and pulling.
