Job Summary

We are seeking a dedicated and compassionate Patient Care Provider to join our team at Trinity Health. As a key member of our care team, you will provide direct patient care and perform specified procedures under the supervision of a registered nurse or licensed practical nurse.

Responsibilities
  • Provide direct patient care according to age-specific guidelines and established policies and procedures.
  • Communicate patient-related data and requests to appropriate team members.
  • Assist in meeting patients' nutritional needs.
  • Assist with ambulation, including the use of walkers, canes, and prostheses.
  • Maintain a safe environment by adhering to hospital policies and procedures for isolation techniques, use of restraints, and falls prevention.
  • Report any unsafe conditions to the appropriate personnel.
  • Document and report care delivered per policy.
  • Perform other related duties as assigned or as workload or staffing variations require.
Requirements
  • High school diploma or equivalent required.
  • CNA license preferred.
  • CPR certification on hire or ability to attain within 60 days.
  • Experience in patient care preferred.
  • Ability to respond to a flexible and changing workload of patient assignments.
  • Ability to perform as a team member, supporting a patient-focused environment in all interactions with patients, families, visitors, and volunteers.
