Key Responsibilities:
- Conduct thorough physical and psychological evaluations and document patient histories.
- Provide essential healthcare treatments.
- Prescribe medications and order medical/psychological tests.
- Assess patient conditions and establish suitable treatment plans.
- Address patient needs with empathy, care, and professionalism.
- Independently perform comprehensive assessments for patients with complex health issues.
- Order and interpret diagnostic tests based on patient-specific needs.
- Implement treatment strategies to ensure patient stability, including monitoring treatment effectiveness.
- Facilitate Medication Services within applicable programs.
- Conduct mental health assessments and monitor medication side effects.
- Educate patients on medication effects and their relationship to mental health and substance use disorders.
- Stay informed on best practices for addiction and mental health treatment.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Oversee care coordination activities, including communication with external providers.
- Develop and approve standing orders as permitted by state regulations.
- Direct evidence-based self-management support strategies.
- Work collaboratively to address the needs of diverse populations.

Qualifications:
- Preferred experience in community health settings.
- Valid nursing license in good standing.
- Federal DEA license required.
- Strong interpersonal skills to connect with patients and healthcare teams.
- Cultural sensitivity to serve a diverse patient population.
- Willingness to obtain additional licensure as needed.

Educational Background:
- Graduate of an accredited nursing program and Nurse Practitioner Program.
- Licensed as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in the state.
- Minimum of two years of experience as a Nurse Practitioner.
