Requirements:
  • A Doctor of Optometry (OD) degree from an accredited institution
  • A valid state optometry license
  • An Ophthalmologist (MD or DO) degree from an accredited school and valid state ophthalmology license (if applicable)
  • A General Liability Certificate
  • Professional Liability Insurance Certificate
