Job Summary

We are seeking a Registered Dietitian to join our team at NYC Health + Hospitals/Coler. As a Registered Dietitian, you will be responsible for providing nutritional care to patients in our Long Term Acute Care Hospital and Skilled Nursing Facility. You will work closely with our interdisciplinary team to develop and implement individualized nutrition care plans, and provide education to patients and their families.

Key Responsibilities
  • Conduct initial and periodic assessments of patients' nutritional needs
  • Develop, implement, and revise nutrition care plans
  • Formulate discharge plans for continuity of care
  • Follow up on patients' nutritional status and revise care plans as needed
  • Interpret and implement diet orders, including enteral nutrition
  • Participate in comprehensive care plan meetings
  • Provide nutrition education to patients and their families
  • Submit daily/weekly/monthly reports of productivity
  • Participate in staff meetings, in-services, and performance improvement meetings
Requirements

To be considered for this role, you must have a strong background in nutrition and dietetics, with a focus on long-term care. You must also be registered by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or certified by the New York State Education Department (NYSED) as a Dietitian-Nutritionist.
