Position Overview:

We are seeking a Physical Therapist (PT) who will play a crucial role in our home health care team.

Key Responsibilities:

  • Assessing and coordinating Physical Therapy services for patients.
  • Evaluating patients' physical capabilities and collaborating with families to provide necessary training and assistive devices.
  • Working alongside other healthcare professionals to set rehabilitation and maintenance objectives.

Essential Duties:

  • Conducting evaluations to determine patients' Physical Therapy needs and delivering services based on authorized medical orders.
  • Performing clinical assessments to identify functional limitations and developing a tailored Plan of Care.
  • Educating patients, families, and caregivers on the use of assistive devices and appropriate therapy techniques.
  • Monitoring and documenting patients' responses to treatment and any changes in their condition.
  • Collaborating with the healthcare team to facilitate quicker recovery and manage ongoing disabilities.
  • Maintaining regular communication with nursing staff to discuss patient progress and any significant observations.
  • Reassessing patients as necessary to track progress and updating treatment plans accordingly.
  • Identifying and addressing any operational or safety issues with equipment used in patient care.
  • Providing comprehensive discharge summaries upon completion of services, detailing care outcomes and recommendations for follow-up.

Qualifications:

  • Graduation from an accredited Physical Therapy program recognized by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.

Experience and Skills:

  • A minimum of one year of experience as a Physical Therapist in an acute care environment; two years preferred, with home care experience being advantageous.
  • Demonstrated knowledge of current Physical Therapy standards and practices.
  • Exceptional assessment and technical abilities.
  • Organizational skills and cultural competence.

Licensure:

  • Must hold a current Physical Therapist license in New York State.
