We are seeking a skilled Home Health Registered Nurse to join our team at AHN Healthcare@Home. As a home health registered nurse, you will play a vital role in delivering high-quality patient care in the comfort of patients' homes.

Our home health registered nurses work collaboratively with our interdisciplinary team to develop and implement patient care plans that address the unique needs of each patient. Key Responsibilities:

  • Develop and implement patient care plans in collaboration with the interdisciplinary team
  • Provide high-quality patient care in the comfort of patients' homes
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements:

  • RN license in the state of employment
  • Graduate from an approved school of nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
