Job Responsibilities- Perform regular home visits to assess client needs and provide personalized care.
- Develop and implement comprehensive care plans in collaboration with healthcare professionals.
- Administer medications, wound care, treatments, and procedures as needed.
- Monitor client conditions, reporting changes to Clinical or Client Services Manager.
- Document observations, interventions, and evaluations accurately using a touch pad tablet.
- A current license as a Registered Nurse in Massachusetts.
- Graduation from an accredited and approved nursing program.
- One year of recent prior clinical experience as a licensed RN.
