Key Responsibilities- Facilitate patient management throughout hospitalization, ensuring seamless communication between patients, families, and healthcare teams.
- Participate in patient management rounds and patient-centered meetings to discuss patient progress and develop effective care plans.
- Identify potential delays and resolve issues with appropriate departments to optimize patient outcomes.
- Collaborate with physicians to refer patients for Physical Therapy, nutrition, speech, and swallow services as needed.
- Serve as an in-patient liaison, planning, assessing, implementing, and evaluating patient care in collaboration with the healthcare team.
- Provide education and support to patients and families regarding their care and discharge plans.
- Document the case management process in the medical record, ensuring accurate and timely documentation of patient progress.
- Bachelor's Degree in Nursing, required.
- Current license to practice as a Registered Professional Nurse in New York State.
- Case Management Certification, preferred.
- Minimum of one (1) year related experience, required. Experience in case management and clinical pathways, variance analysis and trending, quality management/utilization review, and home care/discharge planning, preferred.
