Job Summary

This role plays a vital part in the patient care journey, contributing to the overall success of our clients. The primary responsibilities include participating in the Admissions department, performing telephonic intake calls with patients, and providing medical social services to clients in their home environment.

Key Responsibilities
  • Perform telephonic registration and social work assessment calls with new patient admissions.
  • Assist the Admissions team with other intake duties, such as calling physicians and data entry, to ensure timely patient admissions.
  • Identify patients in need of social work services to set them up for successful outcomes, including prevention of re-hospitalization and improvement in health and physical status.
  • Provide ongoing assessment of needs and participate in discharge planning.
  • Use community resources and participate in care coordination activities to support clients.
Requirements

To be successful in this role, you will need a Master's Degree in Social Work from a Council on Social Work Education-accredited program and a current license. Excellent organization and communication skills are essential, as well as a minimum of one year of social work experience in a healthcare setting.
