We are seeking a skilled Physical Therapist to join our team at JAG Physical Therapy. As a Physical Therapist, you will play a crucial role in providing high-quality patient care and contributing to the growth and success of our company.
Job SummaryThe ideal candidate will have a Bachelor's or Master's degree in Physical Therapy and be licensed to practice in the state where our facility is located. A Doctorate degree is preferred. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. They must also be able to maintain confidentiality and adhere to all applicable laws and regulations.
Responsibilities- Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care.
- This includes, but is not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
- Additionally, records daily billing and follows department guidelines for productivity.
In addition to these responsibilities, the Physical Therapist will be expected to:
- Prepare for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses.
- Select and administer tests and measures.
- Synthesize evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
- Establish treatment plans based on evaluation results, goals, and best practice.
- Collaborate with interdisciplinary teams, patients, and patient support systems.
- Assess and re-assess need for modifications to plan of care and goals.
- Identify need for consultations and/or refers the patient to another health care professional or health care services.
- Initiate and complete discharge plans.
- Provide education/training to patients and/or patient support systems.
- $7,500 sign-on bonus
