- Collaborate with Healthcare Team: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive and coordinated care.
- Coordinate Patient Care: Oversee the delivery of patient care, including the management of home health aides and other support staff.
- Document Patient Progress: Accurately and thoroughly document patient progress, including medical history, treatment plans, and outcomes.
- Ensure Quality Care: Maintain high standards of patient care, adhering to regulatory requirements and CenterWell Home Health policies and procedures.
- Diploma, Associate, or Bachelor Degree in Nursing
- Minimum of One Year Nursing Experience
- Strong Med-Surg, ICU, ER, Acute Experience
- Home Health Experience a Plus
- Current and Unrestricted Registered Nurse Licensure
- Current CPR Certification
- Strong Organizational and Communication Skills
- Valid Driver's License, Auto Insurance, and Reliable Transportation
