Job Summary

Interim HealthCare Home Health and Hospice is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Case Manager. As a Home Health Nurse, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Deliver Skilled Nursing Care: Conduct comprehensive patient assessments, provide wound care, administer medications, and offer other skilled services to patients in their homes.
  • Provide Patient Education: Educate patients and their families on various diagnoses, ensuring they can effectively manage their conditions at home.
  • Facilitate Communication: Coordinate with the patient's care team, including family members, therapists, social workers, home health aides, and physicians, to ensure seamless care.
  • Document Services: Accurately and timely document all services provided to patients.
Requirements
  • Registered Nurse (RN) License: Active, unrestricted license in the State of Michigan.
  • Clinical Experience: At least one year of clinical experience as a Registered Nurse.
  • CPR Certification: Current CPR certification.
  • Transportation: Reliable transportation with a valid state driver's license and current car insurance.
Preferred Qualifications
  • Home Health Experience: One year of experience in home health care.
  • Community Health Experience: Experience in community health and a wide range of medical-surgical experience.
