Overview

We're seeking a dedicated Registered Nurse to join our team at Enhabit Home Health & Hospice. As a national leader in home-based care, we're committed to expanding what's possible for patient care in the home.

Our facility-based RNs enjoy providing care to patients within the beautiful confines of assisted living facilities, where they reside. This role offers 1:1 patient care, the ability to follow each patient from start to discharge, and the opportunity to work collaboratively with facility wellness staff.

Responsibilities
  • Supervise, evaluate, and provide nursing services to clients in various senior living communities
  • Work collaboratively with facility wellness staff to ensure strong lines of communication and overall continuity of care
  • Consult with physicians and office staff as needed to ensure accurate patient care
  • Perform accurate OASIS collection, ensure medication profiles remain current, and report lab values to physicians in a timely manner
Qualifications
  1. Currently licensed as a Registered Nurse (RN) in the state of employment
  2. A minimum of one year of clinical experience is preferred
  3. Demonstrate knowledge and skill in current nursing practice
  4. Possess a valid state driver's license and automobile liability insurance
