Grand View Health is seeking a highly skilled and compassionate Nurse Practitioner, Palliative Care to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to ensure the best possible outcomes for our patients.
Responsibilities
The successful candidate will be responsible for:
- Providing comprehensive health care services to adult and geriatric patients, including assessment, diagnosis, and treatment of acute and chronic conditions.
- Developing and implementing individualized care plans to meet the unique needs of each patient.
- Collaborating with our interdisciplinary team to ensure seamless care coordination and communication.
- Conducting regular patient assessments and evaluations to monitor progress and adjust care plans as needed.
- Documenting patient information and maintaining accurate and up-to-date records.
Qualifications
To be considered for this role, you must have:
- A Master's Degree in Nursing or a related field.
- Over 1 year to 3 years of experience as a Registered Nurse or Nurse Practitioner in a home care setting.
- A valid Nurse Practitioner license and Registered Nurse license in Pennsylvania.
- Current BLS certification.
- A DEA license.
- A valid driver's license.
