Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team at Wexford Health Sources. As an LPN, you will play a vital role in providing high-quality patient care in a correctional setting.

Key Responsibilities:

  • Assist in the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.
  • Direct and guide patient teaching activities under the guidance of Registered Nurses.
  • Utilize electronic medical record systems to document patient information and track progress.
  • Collaborate with healthcare teams to develop and implement individual treatment programs.

Requirements:

  • Currently licensed as an LPN in the state of Illinois.
  • Current CPR certification from a recognized organization.
  • Graduate from an accredited School of Nursing.
  • One year of clinical experience, preferably in a correctional setting.
