Comprehensive Maternal Healthcare Manager

We are seeking a dedicated and compassionate Perinatal Health Home Care Manager to join our team at the University of Rochester. In this role, you will work closely with patients, healthcare providers, and community agencies to provide comprehensive care management services.
Your primary responsibility will be to assess patient needs, develop and implement care plans, and provide face-to-face contact with enrolled participants. You will also collaborate with health, behavioral health, and social service providers to ensure seamless care coordination.

Key Responsibilities- Assess patient needs and develop care plans in collaboration with healthcare providers.
- Provide face-to-face contact with enrolled participants, including home visits and telephonic contact.
- Collaborate with health, behavioral health, and social service providers to ensure seamless care coordination.
- Focusing on serving the most complex, high-utilizing patients who require comprehensive care management services.
- AAS degree with a minimum of three to five years' relevant experience or five-eight years' experience in providing outreach and case management services to women of reproductive age.
- Considerable experience in providing outreach and care management services for a diverse population of childbearing women with co-morbidities within the Rochester community.
- Strong interpersonal skills in engaging patients in case management services.
- Strong organizational skills, capacity to navigate and document in electronic systems, utilize tracking platforms, and communicate concisely orally and in written form.
The University of Rochester offers a competitive salary range of $41,745.60 - $56,347.20, depending on experience. Additionally, you will receive a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 403(b) retirement plan.
