Job Title: Clinical Pharmacist

White Plains Hospital is seeking a highly skilled Clinical Pharmacist to join our WPH Cares team. As a Clinical Pharmacist, you will play a critical role in optimizing pharmacological therapy for patients in an outpatient setting.

Job Summary

Under the supervision of the Medical Director of WPH Cares, the Clinical Pharmacist will collaborate with healthcare providers to provide evidence-based pharmacotherapeutic recommendations, focusing on improving patient outcomes, enhancing medication safety, and preventing hospital readmissions. The ideal candidate will have a strong background in comprehensive medication management and experience working in a telehealth or hybrid healthcare setting.

Key Responsibilities
  1. Perform comprehensive medication reviews and medication reconciliation through telehealth and occasional in-person visits.
  2. Provide recommendations to prescribers regarding optimal medication regimens, focusing on evidence-based guidelines for chronic conditions such as heart failure, COPD, diabetes, hypertension, and others.
  3. Ensure medications are tailored to individual patient needs, considering factors such as comorbidities, drug-drug interactions, and patient preferences.
  4. Assess the appropriateness of drug therapy, including dose, route, frequency, and duration, adjusting as necessary to improve therapeutic outcomes.
  5. Provide patient education on medication adherence, safety, and appropriate use, including OTCs and herbal supplements.
  6. Collaborate with physicians, healthcare providers, and the WPH Cares team to develop and implement care plans, aiming to optimize medication regimens.
  7. Promote clinical pharmacy development by providing leadership and education to pharmacy staff, including telehealth-based interventions.
  8. Identify and manage drug therapy problems, including inappropriate therapy, safety concerns, and cost-effectiveness issues.
  9. Conduct follow-up evaluations and consultations to ensure clinical recommendations are followed and goals of therapy are achieved.
  10. Assist with data collection, analysis, and reporting related to medication management and patient outcomes.
  11. Foster collaboration and trust among colleagues and departments, ensuring a smooth transition of care between hospital, telehealth, and home settings.
  12. Promote a culture of cost containment and efficiency in medication therapy.
  13. Participate in interdisciplinary team meetings, case discussions, and process improvement initiatives to enhance patient care.
  14. Maintain compliance with state and federal pharmacy laws, hospital policies, and telehealth protocols.
  15. Complete relevant competencies and training as required by the Director of Pharmacy.
  16. Collaborate with inpatient clinical pharmacists to provide continuity of care.
  17. Perform other related duties as assigned by the Medical Director and Clinical Manager of WPH Cares.
Requirements
  • Doctor of Pharmacy (Pharm.D.) from an ACPE-accredited school of pharmacy.
  • Active and unrestricted New York State pharmacist license.
  • Minimum of 2 years of experience in comprehensive medication management or ambulatory care (excluding clinical rotations).
  • Experience in a telehealth or hybrid healthcare setting preferred.
  • Certification in Board of Pharmacy Specialties (BPS) or Board Certified Ambulatory Care Pharmacist (BCACPs) (preferred).
  • Certification in New York State Collaborative Drug Therapy (CDTM) is preferred.
  • Familiarity with federal and state pharmacy laws, telehealth protocols, and hospital-based pharmacy operations.

  • Adaptability to work in a dynamic hybrid role with both remote and on-site responsibilities.

  • Demonstrates cultural sensitivity and the ability to engage with diverse patient populations.
Work Environment
  • Ability to perform duties in both a home office and in-person hospital or clinic setting.
  • Frequent use of telehealth platforms and technology for patient consultations.
  • Ability to focus on detail-oriented tasks and make clinical decisions with minimal supervision.
  • Occasional travel required to hospital or clinic locations for in-person patient consultations or meetings.
  • Must be physically able to remain stationary or move about within office and hospital environments.
  • May be exposed to chemicals necessary for pharmacy operations; all safety precautions as outlined in the hospital's SDS database must be followed.
