Key Responsibilities- Conduct initial assessments and screenings to identify high-risk families and develop personalized support plans.
- Provide home-based prevention services, focusing on supporting parent-child relationships, attachment, and family enhancement.
- Collaborate with community agencies and healthcare providers to ensure seamless coordination of services.
- Develop and maintain client records, ensuring accurate and timely documentation of services provided.
- Participate in regular supervision and training sessions to enhance skills and knowledge.
- Attend and contribute to team meetings, conferences, and workshops to stay updated on best practices and industry developments.
- Make referrals to other community resources as needed, ensuring families receive comprehensive support.
- Conduct home visits, assessing family needs and providing case management services to promote self-sufficiency.
- Utilize child development curriculum and play-based interventions to support positive parent-child relationships.
- Establish and maintain effective relationships with families, community partners, and internal stakeholders.
- Perform other related duties as assigned, ensuring the highest level of service delivery.
- Bachelor's degree in Social Work or a related behavioral health field.
- Two years of experience in a related field, preferably working with at-risk children and families.
- Excellent communication and interpersonal skills, with the ability to establish trusting relationships.
- Knowledge of family life skills, infant/child development, and cultural sensitivity.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Flexibility in scheduling, with the ability to work limited nights and weekends.
- Bilingual Spanish preferred, but not required.
- Valid Arizona driver's license and reliable vehicle.
- Ability to obtain a Level One fingerprint card and meet agency liability insurance requirements.
- Ability to sit for extended periods, stand, and walk short distances.
- Must be able to climb stairs and move through facilities and homes.
- Position involves interaction with children, including lifting up to 40 pounds.
- Ability to use office equipment and machines, with normal manual dexterity and eye-hand coordination.
