Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LPN to join our team in the Natrona Heights area. As a Private Duty Nurse, you will provide one-to-one care to patients in their homes, assisting them with daily living activities and promoting their overall health and well-being.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Assess patients' needs and develop individualized care plans to meet their unique requirements.
  • Collaborate with patients' physicians and other healthcare professionals to ensure comprehensive care.
  • Document patient progress and communicate with patients' families and caregivers.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current LPN licensure in the state of Pennsylvania.
  • Minimum of 1 year of experience as a Private Duty Nurse or in a similar role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied shifts, including evenings and weekends.
