Position Overview

The Licensed Practical Nurse (LPN/LVN) plays a crucial role in delivering and documenting skilled nursing care under the guidance of a Registered Nurse. This is done in accordance with the established care plan and physician's orders for each patient, while maintaining confidentiality and professional boundaries at all times.

Key Responsibilities
  • Deliver and coordinate high-quality patient care in alignment with physician directives.
  • Continuously monitor and assess patient conditions, reporting any changes to the supervising nurse or physician as necessary.
  • Document all nursing activities, assessments, and care coordination promptly and accurately.
  • Participate in the development, implementation, and revision of nursing care plans.
  • Take appropriate nursing actions based on patient assessments to achieve desired health outcomes.
  • Recognize and respond to changes in patient needs, implementing interventions to mitigate risks.
  • Uphold personal and professional accountability by adhering to company policies, state and federal regulations, and the Nurse Practice Act.
  • Utilize infection control measures to ensure safety for both staff and patients, following OSHA standards.
  • Educate patients and their families about disease processes, self-care techniques, and preventive strategies.
  • Maintain professional knowledge and skills through ongoing education and training.
Qualifications
  • Graduate from an accredited nursing program.
  • Possess a current, unrestricted state license as a Licensed Nurse.
  • Hold a valid CPR certification.
Preferred Experience
  • Six months of recent nursing experience in a clinical setting.
  • Experience in home health care is advantageous.
Physical Requirements
  • Ability to communicate effectively in English, both verbally and in writing.
  • Capability to travel and lift up to 50 pounds.
  • Prolonged periods of walking, standing, bending, and reaching may be required.
Work Environment

The role may involve exposure to various environmental conditions, including allergens and infectious diseases. Employees must be prepared to function in diverse settings and respond to emergency situations as needed.
