Job Description

**Job Summary:**

BAYADA Home Health Care is seeking a dedicated and compassionate Registered Nurse (RN) to provide exceptional home health nursing care to adult and geriatric patients in their homes.

**Responsibilities:**

  • Develop and implement personalized care plans in accordance with patient needs and goals
  • Conduct home visits to assess patients' conditions and provide necessary care and interventions
  • Administer medications, perform wound care, and provide treatments and procedures as ordered by physicians
  • Monitor patients' conditions and report changes to Clinical or Client Services Manager
  • Document observations, interventions, and evaluations using a touch pad tablet
  • Collaborate with healthcare teams to ensure seamless care coordination and case management

**Requirements:**

  • Current license as a Registered Nurse in Massachusetts
  • Graduation from an accredited and approved nursing program
