Pediatric Home Health Nurse

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team at Phoenix Home Care and Hospice. As a nurse, you will have the opportunity to make a profound impact on the lives of children and young adults with specialized health care needs.
About the Role
- Deliver individualized nursing care to patients based on their unique needs and treatment plans.
- Assist in the development, review, and revision of the plan of care.
- Build strong emotional connections with patients and their families to offer a supportive and reassuring presence.
- Monitor and record patient's condition and document provided care services via electronic charting.
- Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements
- Active Licensed Practical Nurse or Registered Nurse License.
- Problem-solving skills and ability to multi-task.
- Excellent Communication Skills.
- Positive, Professional attitude.
