We're looking for a compassionate and skilled Home Care Assistant to join our team at WelbeHealth in Menlo Park, CA. As a Home Care Assistant, you will play a vital role in providing personalized care and support to our participants in their homes.
Key Responsibilities:
- Provide hands-on personal care and assistance with activities of daily living (ADLs) to our participants, including bathing, dressing, grooming, and toileting.
- Assist with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
- Measure and document participants' vital signs as instructed.
- Aid with light meal preparation and feeding in participants' homes.
- Help participants with safe transfer in and out of their homes and act as a medical escort on vehicles and to medical appointments as assigned.
- Assist the Rehabilitation department by carrying out a home exercise program as indicated in the care plan developed by the IDT.
- Provide translation services as needed, if qualified.
