About the Job

We are seeking a skilled and compassionate occupational therapist to provide home health therapy services to adults in our assisted living communities.

This role involves evaluating and treating patients with a wide range of diagnoses and disabilities, with a focus on balance and fall prevention, therapeutic exercises for functional strengthening, patient and family education, mobility, core strengthening, and pain management.

Responsibilities
  • Evaluate and treat patients in their homes, including assisted living communities
  • Develop and implement individualized treatment plans to meet patients' needs
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care
  • Provide education and training to patients and families on therapeutic exercises and self-care techniques
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Bachelor of Science or Arts degree in Occupational Therapy from an accredited program
  • Successful completion of National Certification Examination for Registered Occupational Therapist
  • Current state licensure
  • One year of home health experience as an OT (preferred)
