Job Title: Physical Therapist (Home Health)

LHC Group is seeking a skilled Physical Therapist to join our team in providing high-quality, patient-centered care in the comfort of patients' homes.

About the Role:
  • Assess and evaluate patient care needs related to functional status, mobility, endurance, and pain management.
  • Develop and implement individualized treatment plans to improve patients' well-being and quality of life.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families to promote independence and self-care.
Responsibilities:
  • Evaluate patients' functional status and physical therapy needs, assisting physicians in developing therapy plans of care.
  • Observe and report patients' responses to treatment and any changes in their condition to supervising nurses and physicians.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards of practice.
  • Instruct and inform participating healthcare team members, patients, and families regarding therapy plans, functional limitations, and progress toward goals.
Requirements:
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, with access to a dependable vehicle.
