Responsibilities- Provide physical therapy services to patients in a Skilled Nursing Facility setting
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Collaborate with other healthcare professionals to ensure comprehensive care
- Maintain accurate and detailed records of patient progress and treatment plans
- Bachelor's, Master's, or Doctorate degree in Physical Therapy
- Current licensure to practice as a Physical Therapist in the state
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Excellent communication and interpersonal skills
