Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team at Centra Health. This role will provide leadership in collaborating with the interdisciplinary team, planning patient care, and promoting a culture of quality and service.

About the Role
  • Estimated Salary: $85,000 - $110,000 per year, based on experience and qualifications
  • Promote and sustain a culture reflective of Centra's best practices for quality and service
  • Lead, communicate and collaborate with the interdisciplinary team to plan patient care
Responsibilities

The successful candidate will be responsible for:

  • Practicing under the tenets of Centra's Professional Practice Model
  • Planning, coordinating, managing, and documenting the care provided to assigned patients
  • Delegating to care providers according to the ANA Principles of Nurse Staffing
  • Thinking critically as evidenced by appropriate clinical decision-making and prioritization
Qualifications

To be considered for this role, you must have:

  • A current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse
  • Graduate of an accredited Nursing Program
