Job DescriptionAs a Home Healthcare Provider - PRN/Part Time, you will conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols. You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
Key Responsibilities- Conduct home visits and assess patients' health, medical history, and risk factors.
- Develop and implement personalized care plans to improve patient outcomes.
- Collaborate with healthcare teams to ensure seamless care coordination.
- Provide education and support to patients and their families.
- Document patient information and care plans accurately and efficiently.
- Master's Degree in Nursing or a related field required.
- Current RN and NP licensure in the state of practice, including prescription authority.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Minimum 1 year experience as a Nurse Practitioner.
- Excellent communication and interpersonal skills.
- Ability to travel and work in a variety of settings.
As a Home Healthcare Provider - PRN/Part Time, you can enjoy a competitive hourly rate of $90.00 or $100.00 based on county, plus mileage reimbursement and no-show payment for booked assessments. You will also have opportunities for professional growth and development within our organization.

LocationThis role is based in Lagrange, Georgia, and may require travel to various locations within the region. You will have the opportunity to work in a variety of settings, including home, skilled nursing facilities, and telehealth environments.
