Perinatal Care Coordinator

The Perinatal Navigator at Greenwood County Hospital Board plays a vital role in providing comprehensive support to pregnant individuals and new mothers.
This position requires a strong understanding of perinatal care and the ability to establish trusting relationships with clients. The ideal candidate will have excellent communication skills and be able to work effectively with a multidisciplinary team.
Key Responsibilities:
- Build and maintain relationships with pregnant individuals and new mothers.
- Provide ongoing support, education, and advocacy to ensure comprehensive and coordinated care.
- Collaborate with the healthcare team to assist clients in setting personal goals and obtaining necessary care.
Requirements:
- Associates Degree in Human Services, Sociology, or Psychology required; Bachelor's Degree preferred.
- CHW certification and perinatal training within one year.
Work Environment:
