The Rheumatology Specialty Nurse is a key member of the Nemours healthcare team, responsible for delivering high-quality patient care in a specialized area of nursing practice. This role requires a strong partnership with providers and other interdisciplinary team members to coordinate patient care through various activities, including phone triage, scheduling, insurance authorization, and communication with families through secure messaging.
Key Responsibilities- Participate in patient and family education, providing information on common rheumatology diagnoses and medications in clinic or via phone.
- Utilize the nursing process as a basis for providing and evaluating nursing care, ensuring patient needs are met and care is delivered in a timely and effective manner.
- Perform ongoing assessment and evaluation of patient progress, revising the interdisciplinary plan of care as needed.
- Identify patient/family needs for further resources and initiate appropriate teaching and/or referrals.
- Participate in responding to patient phone calls, explaining treatment regimens to patients/families after discussion with the physician or advanced practice provider.
- Share clinical knowledge with other members of the healthcare team, collaborating with the Nursing Education Department to disseminate subspecialty knowledge to nursing staff.
- Act as a liaison between families and members of the healthcare team, ensuring seamless communication and coordination of care.
- Utilize specialized knowledge and technical skills in nursing practice related to the subspecialty, adhering to the Nurse Practice Act, hospital policy, and protocol.
- Participate in the development of standards of care for the specialty patient population, ensuring high-quality care is delivered consistently.
- Recognize and identify own learning needs, taking responsibility for continual development and striving to increase knowledge and set goals for professional growth.
- Contribute to departmental and hospital programs for Quality/Process Assessment and Improvement, identifying opportunities to enhance services, making recommendations, and implementing actions as appropriate.
- BSN required
- Active Delaware (or compact state) Registered Nurse Licensure Required
- American Heart Association BLS required
- Pediatric nursing experience required, prior Rheumatology nursing experience preferred
