PRIMARY DUTIES:- Deliver physical therapy interventions in accordance with the physician's prescribed care plan, encompassing evaluation, treatment, and education for patients and their families.
- Develop and oversee individualized physical therapy programs, working in collaboration with a multidisciplinary healthcare team.
- Assess patient requirements for supplementary services and coordinate necessary referrals.
- Engage in discharge planning and compile comprehensive progress documentation.
- Facilitate in-service training sessions and participate in peer review activities.
- Oversee and assess the performance of physical therapy assistants, ensuring adherence to regulatory standards.
- Graduation from an accredited physical therapy educational institution.
- Valid licensure to practice as a physical therapist.
- A minimum of two years of professional experience, ideally in community or home health environments.
- Strong communication and organizational abilities.
- Current certification in CPR.
- Possession of a valid driver's license and access to dependable transportation.
