Curana Health is a leading provider of value-based primary care services for the senior living industry. We are seeking a skilled Healthcare Professional to join our team as a Medical Practitioner in a Post-Acute Care setting.

Key Responsibilities
  • Administer direct patient care services, conducting clinical interventional and episodic visits, and re-evaluating care as needed.
  • Provide ongoing evaluation and treatment of chronic, co-morbid illnesses, as medically appropriate.
  • Evaluate and treat residents for changes in condition, reviewing medications, orders, and diagnostic tests.
  • Coordinate care with collaborating physicians for overall medical management.
  • Conduct Annual Wellness Visits and Annual History and Physicals as requested by facilities.
  • Assist with facility-wide illnesses and coordinate care with other disciplines.
  • Maintain communication with families and attend family conferences when possible.
Requirements
  • Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
  • Masters Degree in Nursing or Physician Assistant Studies.
  • Strong leadership, interpersonal, and problem-solving skills.
  • Excellent written and oral communication skills.
  • Punctuality, organizational skills, and ability to function within a team.
