Job Summary

St. Luke's University Health Network is seeking a skilled Inpatient Registered Nurse or Social Worker to join our team. As a Care Manager, you will play a critical role in coordinating complex discharge planning needs for patients, providing supportive counseling, and facilitating the achievement of desired patient, quality, and financial outcomes.

Responsibilities
  • Assess patient needs, goals, and barriers in relation to clinical condition, psychosocial environment, and socioeconomic resources.
  • Collaborate with multidisciplinary team to plan, implement, and evaluate patient plan of care through rounding or other methods.
  • Design discharge plan with patient, family, or caregivers, and multidisciplinary team, considering patient needs and goals.
  • Proactively consider options such as palliative care, home care, and other services to minimize risk of re-admissions.
  • Work collaboratively with care management team, including social workers, assistants, coordinators, utilization management staff, and director.
  • Facilitate communication among treatment team members.
  • Manage length of stay by identifying and mitigating issues and barriers to care and successful discharge plan.
  • Monitor risk assessment using available tools and implement discharge interventions accordingly.
Requirements
  • MSW or equivalent from accredited school of social work, or RN with current license to practice in Pennsylvania or seeking licensure through reciprocity.
  • At least two years of experience as RN or Social Worker in acute hospital setting, with previous care management experience preferred.
  • Strong critical thinking skills, ability to maintain collaborative and effective working relationships, and assert needs to patients, families, physicians, and other team members.
  • Knowledge of medical terminology, ability to communicate verbally and in written forms, and basic computer skills.
Work Environment

Care Managers will work in various locations across St. Luke's University Health Network, including Allentown, Anderson, Bethlehem, Geisinger St. Luke's Campus, Monroe, Sacred Heart, Upper Bucks, and Warren Campuses.

Shift Details
  • Full-time, 36 or 40 hours per week
  • Part-time, less than 36 hours per week
  • Per Diem, minimum of 2 shifts/month
