Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at VNA & Hospice of the Southwest Region Inc. in Rutland, VT. As a hospice LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional care to our patients and their families.

Job Summary

The Licensed Practical Nurse will be responsible for providing individualized goal-directed nursing care to patients in their homes. This will involve assessing patient needs, developing and implementing care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Responsibilities
  • Provide quality care to assigned patients, including assessing their needs and developing individualized care plans.
  • Carry out the plan of care for each patient, ensuring that the patient and their caregivers are included in the care plan process.
  • Ensure timely and accurate documentation of nursing care provided, including patient progress and any changes to the care plan.
  • Implement medical plans of care, nursing interventions, and procedures as necessary to ensure safe and effective patient care.
  • Evaluate patient responses to care and make recommendations to the case manager as needed.
  • Perform patient and caregiver education to ensure informed decision-making and promote patient autonomy.
  • Identify inter-disciplinary needs of patients and document accordingly, communicating recommendations to the case manager in a timely manner.
  • Coordinate patient care with physicians, providers, and other healthcare professionals to ensure seamless care delivery.
  • Advocate for patient care issues to ensure that overall quality and type of care is sensitive to each patient's unique needs.
Qualifications
  • Licensed Practical Nurse (LPN) license
  • Prior clinical acute care nursing experience required; prior home care experience preferred
  • Valid driver's license, vehicle, and insurance; ability to pass motor vehicle record check, child abuse, criminal record check, and adult abuse check required
