About the RoleWe are seeking a compassionate and skilled Care Manager to join our team. As a key member of our care-focused environment, you will be responsible for providing exceptional support to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Responsibilities- Build Meaningful Relationships: Establish strong bonds with seniors and their families, providing personalized assistance with activities of daily living and attending to individual care needs.
- Notify Management of Changes: Communicate any changes in condition to management and recommend adjustments in care and service levels.
- Assist with Life Skills: Support residents in life-enriching activities as indicated on their individual profiles.
- Participate in ISP Development: Contribute to the development of Individualized Service Plans (ISPs) and monthly updates.
- Ensure Resident Safety: Maintain awareness of resident whereabouts and physically check on them throughout the shift.
- Collaborate with Community Team: Partner with the community team to ensure compliance with OSHA requirements and promote risk management programs and policies.
- Dedication to Serving Seniors: Demonstrated passion for serving seniors with excellent customer service skills.
- Positive Attitude and Flexibility: Ability to perform various duties in service to residents and work effectively in a team environment.
- Qualifications: High School diploma/GED accepted; may be required per state regulations. In states where applicable, must maintain certifications. Must be at least 18 years old. Previous experience working with seniors preferred.
