Ambulatory Care Pharmacist Specialist

Job Summary:
The University of Vermont Health Network seeks an experienced Ambulatory Pharmacist Clinician to join our team. As a key member of our ambulatory care teams, you will apply your advanced practice skills to optimize drug use outcomes in high-risk patient populations.
Responsibilities:
- Apply specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation.
- Participate as a contributing member of ambulatory care teams, including rheumatology, neurology, infectious disease, oncology, nephrology, or primary care.
- Provide clinical, pharmacy, and medication expertise in each area, including policy development, pharmacy operations, prior authorization support, and medication therapy outcomes.
- Develop and implement clinical programs to improve patient outcomes and reduce healthcare costs.
Requirements:
- Doctor of Pharmacy degree and an ASHP-accredited residency program or equivalent experience.
- Licensed pharmacist in the State of Vermont.
- 3 years of advanced practice experience in their area of expertise or equivalent experience in ambulatory care or a clinical outpatient setting.
