Responsibilities:

  • Provide direct patient care to patients with complex medical needs in a long-term acute care setting.
  • Develop and implement individualized patient care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.

Requirements:

  • Current RN license in the state of Wisconsin.
  • At least 2 years of experience in long-term acute care nursing.
  • Current BLS and/or ACLS certification (AHA preferred).
  • Active nursing license per state.
  • Current updated resume and supervisory professional references.
