Role Summary:
The Physical Therapist will provide therapeutic services as outlined in the care plan, adhering to the agency's mission and policies. You will oversee the work of Physical Therapy Assistants and ensure compliance with established procedures.
Qualifications:
Must hold a valid Physical Therapist license in California, have completed an accredited PT program recognized by the American Physical Therapy Association or the AMA, and possess at least one year of relevant experience.
Key Responsibilities:
- Collaborate with physicians to assess patients' functional abilities and develop tailored treatment plans.
- Conduct initial evaluations within 48 hours of referral and perform follow-up assessments every 30 days as necessary.
- Document clinical findings and progress notes accurately.
- Adjust treatment plans in consultation with physicians as needed.
- Engage with patients' families and other healthcare professionals to ensure comprehensive care.
- Coordinate with agency staff to facilitate effective treatment delivery.
- Monitor and report any changes in patients' conditions to the agency.
- Supervise Physical Therapy Assistants, including scheduling and co-signing their documentation.
- Educate patients and families on safe equipment use and home therapy protocols.
- Conduct in-service training for staff as required.
- Participate in quality assurance and peer review activities.
- Maintain effective communication with patients, caregivers, and agency personnel.
- Deliver high-quality care and address any concerns with management.
- Ensure timely completion of all assigned tasks.
- Perform additional duties as assigned.
Additional Qualifications:
- Ability to manage multiple tasks efficiently.
- Physical capability to perform duties, including standing, bending, and lifting.
- Proficient in reading and interpreting patient care instructions.
- Possess reliable transportation for travel to patient locations.
- Maintain current driver's license and auto insurance.
- Comply with annual TB testing, CPR certification, and other necessary documentation.
